Dear Customer:

 Included in your upfront disclosure package are forms that need to be signed and return.   The pages are highlighted to identify which forms all borrowers must sign and return.  In addition to the disclosure information is a form titled “Notice of Incompleteness”.  This form identifies personal information we require from you in order to complete your file.  Please review the information requested and either fax or send it Cathy McDaniel as quickly as possible.  If you’ve already forwarded this information, it will not be necessary to send it again.

 I have included a fax transmittal with my direct fax number for your convenience.  If you would rather mail the information, please use the enclosed UPS or FedEx label.
